1. Go to edit on your website browser tool bar and click on select all and click, now return to Edit and click on copy. Now open up a page in your email account and select paste. This should paste the form into your email. You may have to tweak the spacing but you then can fill it in.
2. Click on the link here which will open up a word document, fill the form in save this document to a file on your hard drive and attach it to an email. Word Doc: you may have to tweak the page margins on your document.
